3.Cultures on Sabouraud’s media with cycloheximide and chloramphenicol.
 
4. Gram’s stain and bacterial culture.

Periodic shedding of the nails may be developmental anomaly and one or more nails may be shed repeatedly. Regrowth may be incomplete or defective. 

Causes of brittle nails are:

The condition is inherited autosomally.Trauma, especially from ill-fitting shoes in persons engaged in any sporting activity may sometimes lead to repeated shedding of the nails. The nails of the great toes are those that are most often affected.
 
In many cases the nail loss is preceded by a subungual haematoma.

  DIAGNOSIS OF CAUSE OF BRITTLE NAIL PROBLEM 

First step in proper treatment of brittle nail is accurate diagnosis of cause of this problem. So we first try to find out cause. We take detailed history, thorough nutritional counseling, physical examination, and examination of hormonal system. After that depending on likelihood of particular cause relevant tests are done.   DETAILED HISTORY

Onset: Sudden/gradual, Primary or Secondary,  Response to previous therapy, Stress (physical or mental), H/O precipitation by some drugs, HT,  trauma, Hypothyroidism / Hyperthyroidism .

  NAIL BITING 

Nail biting is extremely common in children; at least 50% of children exhibit onychotillomania for short periods of time, boys more so than girls. Nail biting is occasionally seen in adults and may be a behavioural abnormality.  sometime  it is familial. Normally, biting is at the tip of the nail and the nail is often bitten right back to the point of separation from the nail bed. Bitten nails are usually very short and irregular. Fine spicules may be left at the edge which cause splits in the nail. Cuticle  biting is similar to nail biting and may result in recurrent attacks of paronychia. Periungual warts are more common in nail biters. Deformities very similar to nail biting can be cause by picking at the nail or by close trimming.
